my guess is that either the drive got physically removed (unlikely, I’d suppose you’d know about that) or that a drive was wiped (i.e. via “dd”) or forcibly pvcreate’d, giving it a new uuid. Now from LVM’s point of view, the PV with the original uuid is missing.
