SUSE 12 SP1 stuck in GRUB prompt after a reboot

We have SUSE Linux Enterprise Server 12 SP1 installed on Power 8 servers and there is issue regarding the same.

In some of our LPARs/machine after a reboot, SUSE LPAR does not boot to OS and instead stuck at grub prompt.

Can someone help to know what happened, what caused this and how to fix this permanently ?
