Swarm Environment Initialization failed

I am facing this issue starting up Swarm Environment through Rancher UI (following the standard document ).
There are thee swarm nodes.

Removed the environment >> removed everything from hosts >>> Recreated the environment >>> Added only 1 host and waited for a couple of minutes >>> Once all services came up >>> Added remaining two hosts.

Its been working fine since then.
